every prospective resident is fully assessed by our Home Manager before admission, forming the basis of ongoing care planning.
each care plan covers preferred daily routine, dietary/health needs, dignity and privacy preferences, a risk assessment, and social interests.
every resident is allocated a key worker as their primary point of contact.
we maintain strong relationships with local healthcare agencies and social services, and residents are encouraged to remain with their existing GP where possible.
The Brooklands is led by an experienced Care Home Manager and a dedicated care team, many of whom have supported residents and families in the local Fulwood community for years.
